個人資料蒐集、處理及利用之告知暨同意書

告知事項

本會為了辦理「2022 TSIA年會」(下稱本活動)，將蒐集、處理及利用您的個人資料（下稱個資），謹先告知下列事項：

1. 蒐集目的：辦理本活動之相關事宜。
2. 個資類別：姓名、連絡電話、電子郵件、公司名稱及職級等記載於活動報名表之個資。
3. 利用期間：至蒐集目的消失為止。
4. 利用地區：中華民國地區。
5. 利用者：本會及其他與本會有業務往來之公務及非公務機關。
6. 利用方式：在不違反蒐集目的的前提下，以網際網路、電子郵件、書面、傳真及其他合法方式利用之。
7. 您得以書面主張下列權利：
   i.   查詢或請求閱覽。
   ii.  請求製給複製本。
   iii. 請求補充或更正。
   iv.  請求停止蒐集、處理或利用。
   v.   請求刪除。
   若有上述需求，請與承辦人員聯繫，本會將依法進行回覆。
8. 若未提供正確個資，本會將無法提供您特定目的範圍內之相關服務。
9. 對本會持有您的個資，本會會按照政府相關法規保密並予以妥善保管

Notification and Consent Regarding Collection, Processing and Use of Personal Information

Notification

In order to organize the 2022 TSIA Convention, Taiwan Semiconductor Industry Association (TSIA) will collect, process, and use your personal information which you have provided or will provide (hereinafter referred to as “the Personal Information”), TSIA would like to inform you about the following matters:

1. Purposes for collection: Activities related to enrollment
2. Classification of the Personal Information: Registered name, phone, e-mail address, company/organization name, job title, etc.
3. Time period of the use of the Personal Information: until the purposes for collecting the information no longer exist.
4. Areas of use of the Personal Information: The territory of the ROC.
5. Users of the Personal Information: TSIA as well as government agencies and non-government agencies that have or will have business relations with TSIA.
6. Way of the use of the Personal Information: Under the condition that there is no excess of the scope of the purposes for which the Personal Information was collected, the Personal Information may be used on the Internet, in e-mail, in documents, in facsimiles, and in other lawful ways.
7. You may exercise the following rights by raising written request(s)any inquiry and request for a review of the Personal Information;
   i.   any request to make duplications of the Personal Information;
   ii.  any request to supplement or correct the Personal Information;
   iii. any request to discontinue collection, processing or use of the Personal Information; and
   iv.  any request to delete the Personal Information.
   If you have any questions about the retention of your personal information, you can contact us.
8. If you do not sign this Notification and Consent Regarding Collection, Processing and Use of Personal Information, TSIA may not be able to offer a complete range of service(s) to you, and may also not be able to protect your lawful rights.
9. TSIA will keep your Personal Information confidential and in proper custody in accordance with the relevant laws and regulations of the ROC.
